G3SKATES LIABILITY WAIVER & RELEASE OF CLAIMS (READ CAREFULLY) By signing this Waiver, I (the “Participant”) and/or the Participant’s Parent/Legal Guardian (if Participant is under 18) acknowledge that roller skating and related activities involve inherent risks, including but not limited to falls, collisions with objects or other skaters, equipment failure, and risks associated with outdoor surfaces, weather, and facility conditions. 1) ASSUMPTION OF RISK I understand and voluntarily accept all risks of injury, illness, disability, or death, and property damage that may arise from participation in skating lessons, practice, demonstrations, or any related activity with G3Skates, whether occurring before, during, or after instruction. 2) RELEASE OF LIABILITY To the fullest extent permitted by law, I release and hold harmless Greyson Glaze, G3Skates, and any assistants, contractors, volunteers, or partners (collectively, the “Released Parties”) from any and all claims, demands, actions, or causes of action arising out of or related to participation, including claims alleging negligence of the Released Parties. 3) MEDICAL ACKNOWLEDGMENT I certify that the Participant is physically able to participate. I understand that G3Skates does not provide medical care. In the event of an emergency, I authorize the Released Parties to seek emergency medical treatment if needed. I agree to be responsible for all costs of medical care and transportation. 4) SAFETY RULES & CONDUCT I agree to follow all safety instructions, rules, and coaching guidance. I understand that failure to follow instructions may increase risk of injury and may result in removal from the lesson without refund. 5) EQUIPMENT & PERSONAL PROPERTY I understand that skating equipment can fail or be improperly fitted. The Participant is responsible for ensuring gear is properly fitted and maintained. I accept responsibility for personal property and agree the Released Parties are not responsible for loss, theft, or damage. 6) WEATHER / SURFACE CONDITIONS I understand outdoor surfaces may be uneven or slippery and weather may change quickly. Sessions may be rescheduled due to unsafe conditions. 7) PHOTO / VIDEO PERMISSION (OPTIONAL) I understand G3Skates may take photos/videos for training or promotional purposes. I may opt out below. 8) PAYMENT / CANCELLATION Payment secures a spot. 24-hour notice is required to reschedule. No-shows may forfeit the session. Weather-related reschedules are allowed. 9) PARENT / GUARDIAN CONSENT (IF MINOR) If the Participant is under 18, the Parent/Legal Guardian must sign and consents to the minor’s participation and agrees to all terms of this Waiver on behalf of the minor. ACKNOWLEDGMENT I confirm I have read this Waiver, understand it, and sign it voluntarily. I understand I am giving up substantial rights, including the right to sue.
